Patient Information

Here’s what to expect upon scheduling your procedure at Fort Wayne Endoscopy Center:

You will be contacted prior to your procedure to confirm your arrival time on the day of your procedure.

Do not eat or drink anything after midnight the day before your procedure. If you are scheduled to arrive at the center after 12 PM, you may drink approved liquids up to 4 hours before your scheduled procedure time.

Please remember: Stop all Aspirin, Ibuprofen, Coumadin, Plavix, Ticlid, blood thinners and iron supplements five days prior to this procedure. You may take Tylenol for pain if needed.

In the mail, approximately 5 days prior to your procedure, you will receive a packet containing the following important documents to read and familiarize yourself with:

  • Fort Wayne Endoscopy Center Patient Information Brochure
  • Fort Wayne Endoscopy Center Patient Right and Responsibilities Brochure
  • Authorization to Treat
  • Informed Consent for gastrointestinal endoscopy
  • Contact information to file a grievance

Original copies will be signed on site.

Bring the following items with you on the day of your procedure:

  • list of medications (prescriptions and over-the-counter)
  • Insurance card(s)
  • Photo ID
  • Advance Directive/Living Will

Due to receiving sedation for your procedure transportation arrrangements must be made for your return home.

Your call will be returned within 1-2 business days, Monday through Friday.
